Physician or Other Clinical Partner
Chronic Care for CPAP - Incremental and Free for Your Patients
- Effective: Compliance improvements in-line with clinical studies, with 4th Quarter 2007 compliance at 93%! (Read this Page for more on these clinical studies and 4Q07)
- Incremental: Chronic Care does not replace the current physician role or traditional DME rental services, set-up or refitting. (In fact, we reinforce your recommended doctor-visit schedule with your patients). Why not educate patients on the option for additional care & support between physician visits?
- Free: Coaching & other chronic care is provided for free to patients of clinical partners (presuming they have qualifying insurance coverage for mask supplies). Why not add this as another benefit of your practice that you can offer to patients?

Traditional DME follow-up is not enough for many patients
The national statistics of compliance rates in the 40-50% range are self-evidence that it is optimistic at best to rely on an equipment supplier who is focused on the initial set-up.
Clinical research studies provide further evidence:
- 14% of 949 typical CPAP patients had follow-up records adequate for analyzing compliance
Hin, etal., Otolaryngol Head Neck Surg, 2007 Read this Page
- Control Group of 50 patients - "treatment as usual": compliance rates only 31%
Bartlett, etal., Sleep, May 2007 Read this Page
Chronic Care support for New or Veteran Patients after the DME set-up & between Physician visits
Luckily the clinical research points to a solution for low compliance rate:
proactive outreach with behavioral therapy and other modes of chronic care:
- Bartlett, etal.: Test Group of 50 patients - behavioral therapy: compliance rates of 77%.
Bartlett, etal., Sleep, May 2007
- 296 CPAP patients receive proactive outreach, "trouble-shooting," and regular feedback to both patient and physician: compliance > 85%
Sin, etal., Chest, 2002
Better yet, the compliance (based on 4+hrs/ night for 5 nights/ week) for patients enrolled in SleepCare for 4th Quarter of 2007 was 93%!!

SleepCare - Augmenting Traditional Follow-Up
While the DME providing the initial set-up plays a vital role, many patients require the additional support that chronic care delivers:
| Traditional Follow-Up |
CPAP Chronic Care |
- Done by general DME
- Connected to CPAP rental
- Focused primarily on initial dispersal & fitting
- Lower priority "part-time" job
- React to patients who call first
- Total patient education & coaching time: 20-40 min.
|
- Done by CPAP chronic care specialist
- Separate from CPAP rental
- Focused on long-term / on-going education, coaching & habituation
- Dedicated staff's "full-time" job
- Proactive outreach to all patients
- Total coaching time: 1.5 hrs in first month, then 15-30 min. each quarter

Educating Patients on a New Support Resource
Many patients don't know about the resources available to them unless their physician educates them on their options.
However, once educated, many patients appreciate the opportunity to get coaching support on an on-going, convenient basis.

Why Follow-Up Often Isn't Enough
The education and training given during this initial set-up is fast and furious, and rarely thorough enough. Then patients are left alone to deal with a very challenging road to recovery (and no one to call for help).
Unfortunately, many patients don't reach out to their DME or even their physician when they have problems. There are the two reasons why they might not call.
First, many patients don't call exactly because they are having problems:
- They might not even realize there is a problem - they just assume it's supposed to feel so awkward (we've even found some people wearing their masks up-side-down without even realizing it).
- Or maybe they've already given up. Obviously they're not going to call if they've given up... but if they are called proactively and get their problems addressed, they might have a change of heart and give CPAP another try.
- Or, they could just lose motivation - not because of a major problem, but because it was just a huge hassle and there was no one to encourage them to keep on going. Our customers tell us that simply knowing someone will be calling them gives them a shot of motivation to stick to the program.
Second, even if they know they have a problem and want to continue CPAP, they might not call because they don't want to make an appointment for something they assume is minor, or they have a hard time scheduling time to come in.
Proactive chronic care along with giving the patient the ability to get help by making a simple phone call are key for many patients compliance. In other words, convenience matters - plain and simple.
- A CPAP-specific, chronic care program to help educate, enable and habituate your patients.
- Long-term, proactive help with education, compliance coaching and trouble-shooting.
- Respiratory Therapists to answer patient questions and motivate compliance.
- Initial month and quarterly reports that assist you in managing the sleep therapy of your patients.
- All that is required is a prescription for the replacing sleep mask supplies.
