Studies Show That Chronic Care Increases CPAP Compliance

Australia Study on Compliance and Coaching

In May of 2008 Woolcock Institute of Medical Research of Australia released a study of the effect of cognitive behavioral therapy (CBT) on CPAP therapy and compliance. (CBT is just a fancy way of saying “coaching”).

One hundred participants were assigned randomly to either the treatment as usual group or to the coaching group.

People in the “coaching” group attended 2 one-hour sessions designed to help them understand both the risks of sleep apnea and, more importantly, the future benefits of continuing treatment.

At one month only 31% of the “treatment as usual” group had used their CPAP for at least four hours a night. (Sadly, that’s not far off from the average compliance rate in America – it’s consistently less than 50%, which isn’t very good odds!).

But the “coaching” group had 77% of participants using their CPAP for at least four hours a night. That is a big difference – more than double the 31% compliance rate of “treatment as usual”!!

Perhaps more importantly, coaching helped keep people from quitting CPAP therapy altogether (and putting their sleep and health at jeopardy).

How much help at staying the course? The study found that 30% of the treatment as usual group refused CPAP treatment, where only 8% of the coaching group refused treatment.

Dr. Delwyn Bartlett, Sleep Psychologist at the Woolcock Institute of Medical Research said, “The results are extremely encouraging, particularly as some or most of this intervention enabled individuals to use CPAP on a nightly basis which is a primary goal of treatment.”

That is pretty good news…and a really good reason to get a coach!

Yale Study on Compliance and Education

In another study, researchers from Yale University and Bridgeport Hospital took 88 CPAP patients and put them into 2 groups: one group that received education and mask acclimatization (which is a technical way of saying they were familiarized with their mask and equipment), and the control group who received only traditional education. All the patients had follow-up at 1, 3 and 6 months.

The study, which was presented at the annual conference of the American College of Chest Physicians – CHEST 2006, found that only 8% of the group who received education and familiarization failed to use their CPAP, while 31% of the control group failed to use it.

The researchers concluded that there was better compliance when patients received education and familiarization. Said Dr. A.E. Petersen, “Education and mask acclimatization prior to initiation titration study may be crucial in helping patients be more compliant and receptive to therapy.”

University of Alberta Study on Coaching

Finally, a study in 2002 from the Departments of Pulmonary Medicine, University of Alberta, Canada conducted a study to determine what the long-term compliance rates were in patients who were enrolled in a comprehensive CPAP program in the community.

Two hundred and ninety six men and women were given a CPAP device equipped with a monitoring chip; patients were called daily the first week and were seen at 2 weeks, 4 weeks, 3 months and 6 months.

At 6 months the average duration of CPAP use was 5.7 hours per night and the percentage of patients using CPAP (at least more than 3.5 hours per night) was 88.5% as compared to the usual ‘less than 50%’ compliance in the United States.

Dr. Irvin Mayers and authors of the study concluded that a “CPAP program consisting of consistent follow-up, ‘troubleshooting,’ and regular feedback to both patients and physicians can achieve CPAP compliance rates of > 85% over 6 months.”
